We Offer
Functional Capacity Assessments
Social Workers are one of the preferred professions to write Functional Capacity Assessments (FCA) for participants with psychosocial disability. FCA’s are a thorough, holistic assessment of a participant’s functional capacity across the domains of social interaction, communication, learning, mobility, self-care and self-management. FCA’s are usually written to assist with application to the NDIS, or when circumstances change to support a plan reassessment.
Level 2 Support Coordination
This support will assist you to build the skills you need to understand and use your plan. A support coordinator will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community.
Level 3 Support Coordination
This is a higher level of support coordination. It is for people whose situations are more complex and who need specialist support. A specialist Support Coordinator will assist you to manage challenges in your support environment and ensuring consistent delivery of service.

RISE ABOVE FOUNDER
Renee Cooke
Renee graduated from Charles Darwin University with her Bachelor of Social Work in 2016. She is a current member of the Australian Association of Social Workers (AASW). Renee is an experienced Specialist Support Coordinator and Report Writer. Renee was previously employed at the Department of Correctional Services where she wrote high level reports for the Courts. Renee is non-judgemental, analytical, and thorough. Renee has experience working with participants living with all types of disability. Her special area of interest is supporting participants with psychosocial disability.
